ProTaper Gold Tackles for Narrow Canals
Case Summary
After determining tooth #19 to have necrotic pulp as well as chronic abscess on the apical side I suggested RCT for an elderly woman aged 66. The challenges in this case are finding a path that allows for a glide route to the working length, canals that have been calcified as well as a tense curvature. Diagnosis
The case concerns a 66-year-old female with a lengthy medical background. Clinically speaking, the tooth has sinus drainage. The tooth showed normal probing depths and no discomfort from percussion, and no reaction to cold.
This tooth was identified as having Necrotic Abcess and Pulp. It was also diagnosed as Chronic.
Access and Shaping
The entrance was easy There was however an uncut pulp stone inside the chamber.
The canals measured 22mm in length and finding my glide path to work length was the main difficulty in this particular. In the beginning, I employed C Files and EDTA to get an even patency. I utilized my handpiece from the endo along and the C files to become more efficient in the procedure.
The other issue was the curvature of MB root. The combination of curvature and calibration creates an unorganized file, which is why I would like to give this portion of the process more time.
Once the glide path had been established, I was sure I had my motor good to go. I utilize a hybrid approach of S1, F2, and then I switch to ProFile 25/.04 and 30/.04.
Obturation and Irrigation
In this particular instance I didn’t activate my irrigants as I do not find them effective in situations that I have narrow canals. However, I prefer to use a 30 gauge ProRinse needle for my irrigation to ensure the maximum needle penetration to ensure more efficient irrigation.
I removed the tooth using hot vertical condensation. Then I sealed the tooth with ThermaSeal’s the Ribbon Plus Sealer.
Restoration
The flowable material was positioned over each orifice to create an orifice blocker and a cavit and sponge were put in. The patient was sent for a return visit to her GP to be restored.
